What are Patient Support Services?

Medical Support Services provide professional, comprehensive, family-orientated services to individuals in the home setting. The role of a Medical Social Worker (MSW) is to evaluate and develop a plan of care personalized to fit the patient’s emotional and social needs. The MSW works in collaboration with the patient, their family, and caregivers to develop a plan of care, while also helping to provide experts with understanding social and emotional factors impacting the patients’ health.

 

Community Support Services help clients face challenges that are not exactly clinical. They often need help acquiring certain life skills or accessing community resources. Community Support Worker (CSW) services support individuals and families with the services and resources to help promote recovery, rehabilitation and resiliency, while addressing goals in the domains of independent living; learning; working; socializing; and recreation.
